New Patients

All new patients need to fill out a Registration Form and a Medical History Form. These can be obtained from reception, and you will need to fill in a separate form for each new patient who wishes to register. To save you time, you can download the forms here and print them off to be filled in at home. You can then bring the completed forms with you when you come in to register.

Your First Visit

When you arrive at our practice we will:

  • Examine your teeth and mouth.
  • Record measurements of your gum health.
  • Screen for oral cancer and, if required, take X‑rays.
  • Take a detailed medical and dental history.

Creating Your Treatment Plan

After we have listened to your concerns and discussed the possible treatments, we will design a personalised plan based on the information gathered at this visit. At your next appointment, we will review the plan with you, answer any questions and explain the expected outcomes before any treatment begins.

Children’s Dentistry

Exempt patients must present their exemption documentation at the first appointment. Our reception team will gladly help with any queries you have about payment exemptions.
